Case Worker Outreach

Lifespan & Gateway Healthcare are currently searching for several Case Workers to join our Outreach team! We have several positions available in Charlestown, RI & Pawtucket, RI. Additionally, we have opportunities working with Children & Families or Adults. Typical responsibilities for these positions include:

·        Provide case management, rehabilitation, crisis interventions, and outreach services in home, office, and community settings to adults or children with mental illness

·        Collaborate with psychiatric & allied mental health teams in assessing diagnosis intervention & treatment planning

·        Conduct substance abuse treatment with clients

·        Provide clinical interventions & linking Clients with community resources, including:

o  Assist Clients in obtaining financial benefits

o  Assist Clients in accessing medical & dental care

o  Assist Clients in choosing, obtaining, and keeping employment

o  Assist Clients in choosing, obtaining, and keeping affordable housing

o  Assist Clients in developing social support networks and participating in social & leisure activities

o  Assist Clients in applying ADL skills in needed real-life environments

·        Conduct comprehensive clinical assessment & develop interventions for substance abuse, sexual abuse, symptom management, and family support & education

·        Coordinate emergency psychiatric services within a timely manner, including documenting interventions, alternatives considered, and response of Client/Client’s Family, and supporting agencies

·        Provide a minimum of 22 hours of direct services with Clients

·        Document progress notes following established procedures

·        Participate in treatment planning

·        Assist with discharge planning

The ideal Candidate for the Case Worker position will possess:

·        Bachelor’s degree in the Human Services field, such as Social Work, Psychology, and Rehabilitation Services, required

·        2 years’ experience in Human Services, preferred

·        Experience working with adults with substance abuse, sexual abuse, or involvement with the criminal justice system, preferred

·        Valid driver’s license & use of vehicle during normal working hours, or other means of transportation to complete major functions of the job

·        Automobile liability insurance coverage for vehicle being used in accordance with state regulations
